Q1:  What is the size of the mounting holes?

The holes are a quarter of an inch. Center to center is one and a quarter inches. Plate size is one and thirteen sixteenths inches.

Q2: Does the brake prevent the swivel from turning as well as the wheel?

Yes it locks securely, both in not rolling and swiveling.

Q3: What are the mounting hole dimensions? (Hole pattern)

The top plates are about 3" square, and have four holes. The screws that come with the wheels are #10 x 1".
